All drivers who hold HGV/PCV entitlements and are over 45 must undergo a D4 medical with a GMC registered Doctor when changing an EU licence to UK group 2 driving licence.
Any GMC registered doctor can complete your medical and you do not have to visit your own GP for this medical. We can still carry out your D4 medical even if you are not registered with a GP surgery in the UK, just write ‘not registered’ on page 1 of the form.

You must be a resident with a permanent address in the UK (that you’ve lived in for at least 185 days) and have passed your driving test in an EU member country. If you are over 45, you must immediately undergo a D4 medical; if you are under 45 you do not require a medical until you reach this age.

For the vision requirement, you must meet 6/7.5 in one eye and 6/60 in the other on a 3 metre snellen chart. If you do wear driving glasses or lenses, you should bring these along with your current prescription to the assessment.
