All PCV and bus drivers must complete a D4 medical when applying for a provisional category D licence.
After your initial medical you need a renewal medical at 45, then every 5 years until it becomes an annual medical from the age of 65. There can be exceptions to this rule though and you should always check the date in column 11 on the back of your driving licence. For renewals DVLA will also send you renewal paperwork by post 56 days before your licence expiry.

There are two main reasons why first time and returning PCV drivers visit our Doctors for the D4 medical; time and money. If you visit your own GP for your PCV medical you will expect to pay over the odds and face a lengthy wait, you’ll also have to pay an optician for the vision test.
With D4Drivers your PCV medical is only £55, includes the eye test and most drivers are seen within a week. Please note if your vision is borderline or deemed a ‘fail’, in the interests of driver safety our doctor will refer you to your optician for this section of the form to be completed.
The speed of our service is important to many professionals who need their PCV Category D Bus licence to be able to legally drive any kind of bus or coach, including single deckers, double decker buses and also bendi buses.
If you passed your driving test in 1997 onwards and want to drive a minibus over 3.5 tonne you require a D4 medical to obtain your provisional D1 licence. If you want to drive a minibus commercially, regardless of your age you need a D4 medical to obtain your D1 licence.
Our doctor will complete your full D4 medical for only £55. Your medical consists of an eyesight test using a Snellen chart, blood pressure checks to ensure your BP meets DVLA criteria and a questionnaire regarding your medical history.
If you do wear driving glasses or contact lenses, you should bring along your current prescription to the appointment. You must meet 6/7.5 in one eye and 6/60 in the other on a 3 metre snellen chart. If you do wear driving glasses or lenses, you should bring these along with your current prescription to the assessment.
Your medical assessment will also include checks for the standards that need to be met by PCV drivers. During your assessment, we will complete your D4 form based on the answers you provide relating to your medical history, medications, and the findings of your examination.
This will include recording your responses on the D4 form to questions regarding a range of health and medical matters, including epilepsy, seizures, diabetes, cardiac issues and many other forms of medical condition, in order to prove your suitability for a category D licence. The whole process of carrying out a PCV medical should usually take around 10-20 minutes but this does depend on your health and medical history.
