    Taxi Medical Form – A copy of the taxi medical form relevant to the council you are applying for. You can find a copy of this form on our taxi medical page here.

    Medical Records – Please check with the council if summary of care records or full medical records are required. If so, we have template letters for you to use and request a printed copy of your medical records here.

    Urine Sample – Please bring a urine sample in a secure container. It is your responsibility to dispose of this safely off site, once the medical has been completed.
